Use the Online Registration Form above or use the registration form and follow instructions.
NOTE: The 2004 fees are subject to taxes requires by French laws.
Confirmation of Registration
Payments may be made by credit card or by personal check payable to CTA/INCOSE 2004. To pay by bank transfer,
please to contact CTA at karin.moens@cta-congres.com for wire tranfer instructions.
Payment is due within thirty (30) days of receipt of registration.
Confirmation letters will be mailed or e-mailed within two weeks of receipt of registration form and payment.
If you do not receive a confirmation letter within three weeks of payment, please contact PMCI by fax, e-mail or telephone.
If payment is made by purchase order, please ensure that correct billing information is furnished in the space provided on the Symposium
Registration Form.
NO PURCHASE ORDERS WILL BE ACCEPTED ON SITE.
Payment in full of all Symposium registration fees must be made before, or at the time of, on-site registration.
Bring your confirmation letter or proof-of-payment to facilitate on-site registration.
Symposium Cancellation Policy
If you enrol and cannot attend, a 50 Euros member processing fee or 117 Euros non-member processing fee will be
retained if a written request is received by CTA by 7 June 2004. For cancellations after that date, a 50% processing fee paid will be retained.
No refunds will be given to registraints who fail to cancel by 16 June 2004, or who do not show up on-site.
In the case that INCOSE must cancel the event, the full event fee will be refunded. Substitutions are allowed at all times.
Annual dues received cannot be refunded.
