This paper describes the activities that were undertaken during a twelve-month study investigating the application of Human Reliability Assessment (HRA) within a major international systems company. It is hoped that this paper might act as a template for other companies wishing to re-evaluate their HRA processes and in order to develop sustainable processes to manage the generation and application of HRA along the engineering life cycle.
